This study explored how rural-practicing clinicians communicate with patients about palliative and end-of-life care when they have close personal relationships with those patients. Interviews with 10 health care clinicians from a rural state were analyzed using reflexive thematic analysis. The overarching theme, “unlocking the diary,” described how familiarity acted as a key to accessing both personal and professional views of patients’ experiences. Three diary entry–like themes emerged: emotional toll, bringing the pages together, and honesty. The findings suggest that prelicensure and postlicensure curricula should teach clinicians how to use familiarity to strengthen communication and support better palliative and end-of-life decision-making.
